Request a VHDF Test
To host a VHDF sanctioned test you need to submit a Test Request Form and a Test Insurance Form. For more information on holding a VHDF sanctioned test, go to the Test Resources page or contact VHDF at testrequest@vhdf.org
You should submit your test request as soon as possible to reserve your date on the calendar. The test sanction fee must be paid 60 days prior to the test date.
Please mail the form and sanction fee payable to VHDF in the amount of $450 to:
