Test Data Entry Form
The Data Entry Form is a MS Excel spreadsheet to be used by the Test Coordinator.
The Test Coordinator should download the Data Entry Form (DEF) and fill out the required fields for each dog entered in your test. In addition, the DEF will create your scorebook labels and mailing labels.

It is recommended that each Test coordinator contact John Smith for instructions and hints on how to use the DEF. Contact John at jgsmith76@gmail.com
As soon as your test is over the completed DEF should be emailed to John Smith.
